Make a Report (Steps)

Report a Fatality or Severe Injury

  • All employers, including the Navajo Nation Government, Enterprises, Entities, Agency Chapters, Schools and Contractors, are required to notify NNOSHA when an employee is killed on the job or suffers a work-related hospitalization, amputation, or loss of an eye.
  • A fatality must be reported within 8 hours.
  • An in-patient hospitalization, amputation, or eye loss must be reported within 24 hours.

STEP 1 Call NNOSHA at (928) 871-6742 during normal business hours 8:00 am to 5:00 pm. If you call outside normal business hours, please leave a message and someone will return your call as soon as possible.

STEP 2 Provide the following information:

  • Business Name and address
  • Names of employees affected
  • Location and time of incident
  • Brief description of the incident
  • Contact person name, phone number, and email

STEP 3 NNOSHA will review provided information and determine next steps to address the situation, which may include an investigation by the Senior Safety Technician or Program Supervisor.
